Job Description As a Remote Chronic Disease Nurse at CVS Health, you will play a vital role in coordinating care for patients with chronic diseases such as diabetes, hypertension, heart disease, and more. You will be responsible for conducting assessments, developing care plans, and providing education and support to patients to help them manage their conditions effectively.
